Plan Description

The CA50 HMO Plan is our most comprehensive plan we have to offer. Anyone can qualify with or without work. Too often, a minor tooth ache can be an indication of a major problem requiring extensive work to save a tooth. The CA50 HMO plan will save you the most out of pocket costs for those major problems like crowns, bridges, dentures, and much more. No one can predict the future but you can prepare for it. Buying the CA50 HMO Plan will not prevent a problem with your teeth but it will significantly reduce the cost of repairing the problem.

As with any HMO, you must designate your primary care dentist at the time you purchase your plan. By clicking on the FIND A DENTIST button above and entering your zip code, you will find a list of providers within your area that accept the CA50 HMO Plan. Later, should you decide to change your dentist, just give us a call and tell us which Dentist you will be using as your primary provider.

Remember, NO WAITING PERIOD means you won’t be told you must wait 6 months or a year before you are covered for a major procedure. You can repair the problem now before it gets worse. NO MAXIMUM means you can use your plan for the full twelve months without concern for reaching a maximum amount of coverage. NO DEDUCTIBLE means you will not be required to pay full price for procedures until before your coverage takes effect. All family members must use the same Network Dentist.

Annual premiums are paid in full when purchasing your plan. There are no monthly payment plans available.

Premium prices include a $20 non-refundable administration fee. Your plan will be activated within two (2) business days and there are no waiting periods for any covered procedures.

Co-Pays for all dental procedures are paid directly to your Dentist.
